Description: This session will focus on the role of schools and programs of public health in a rapidly evolving health care delivery system. The opportunities that are available in various integrated delivery models will be highlighted. Lessons from the use of the Practical Playbook will also be discussed. The role of prevention in various delivery models will be discussed, along with the CDC's 6|18 Initiative, which aims to "accelerate evidence into action to improve the health of all Americans."
